Nursing Home Relocation

Nursing home relocation may be an option for someone who is a resident of a nursing home that wishes to relocate to either independent living in the community or to assisted living. You may be eligible if:

 q   You are 65 years of age or older OR 18 years of age or older and have a disability

 q  The nursing home is currently billing Wisconsin Medicaid for your stay/services

 q  Your stay in the nursing home has been long-term or is expected to be long-term

The alternative programs that may be able to assist you in relocating would include Community Health Partnership, Inc.?s programs or IRIS (Publicly Funded Long-Term Care Page).
