Publicly Funded Long-Term Care
There are four publicly funded long-term care options offered in Eau Claire County. Three of the programs are designed to provide individuals with the services they need to remain living in the community. That may include services so someone can continue to live in their own home, independent apartment or assisted living facility. The fourth option is Medicaid funding for long-term nursing home care called Institutional Medicaid. Community Health Partnership In Eau Claire County, Community Health Partnership, Inc. offers two publicly funded long-term care options. They are Community Family Care and Community Health Partnership. With Community Health Partnership, Inc., you will work with an inter-disciplinary team consisting of a Registered Nurse and a Social Service Coordinator. Your team will assist you in developing your service plan and obtaining the services you need to remain as independent as possible. There are eligibility requirements in order to qualify for these programs. Eligibility is determined through the Aging & Disability Resource Center. IRIS IRIS stands for ?Include, Respect, I Self Direct?. IRIS is a publicly funded, long-term care program in which the participant chooses to self-direct their care plan and services. You will work with an Independent Consultant in using your monthly budget for purchasing services. Your budget is based on the long-term care functional screen that is completed with an Options Counselor at the Aging & Disability Resource Center. Nursing Home Medicaid If your choice is to stay in a nursing home long-term, and need funding assistance to do so, you will need to apply for Wisconsin Medicaid. Eligibility for Publicly funded Long-Term Care The eligibility requirements are the same for both of Community Health Partnership, Inc.?s programs and IRIS. There are two different areas of where eligibility needs to be met. The first is your functional eligibility, which is determined through a tool called the long-term care functional screen. The functional screen looks at your ability to perform the activities of daily living such as bathing, dressing, preparing meals, managing finances, managing medications, driving, etc. This assessment will also provide you with your monthly budget allowance for the IRIS program. The second area of eligibility is financial.
